ENS4Care: Evidence Based Guidelines for Nurses and Social Care Workers for the deployment of eHealth services

From the ENS4Care website:

“Innovative, high quality, safe and cost-effective national healthcare systems are dependent upon policy-makers and stakeholders developing and implementing high-quality eHealth services”
(Sheikh et al., 2011)

This is particularly the case under the current social and economic situation plaguing EU Member States. ENS4Care is designed as response to this need with an ultimate aim of contributing to an evidence based deployment of eHealth services in the EU and Europe. While Member States are individually striving to respond to a growing demand for quality, safety, equity and access, they are at the same time challenged to be innovative with regards to the sustainability of their healthcare system. Furthermore, in order to render health and social protection more adequate and sustainable, there is a need to invest in health and social care staff´s skills and capabilities to support people in need (Social Investment Package, 2013). The identification of these skills together with the exchange of good, innovative, implemented and cost-effective solutions and approaches is increasingly needed. In this context, the policy initiatives set out in the Digital Agenda ensure that the European Commission, closely cooperates with EU Member States, and different stakeholders, are the driving forces to making clear implementation proposals in the field of eHealth services. http://www.ens4care.eu/projects/policy/

mEDucator 3.0 | Open Linked Education - Melina+ (Drupal)

Checking through links yesterday I came across the mEDucator initiative and Melina+ which is part and noticed it is built using Drupal.

There is a thread on Drupal.org on science applications:

I posted in 2009 about the Science Collaboration Framework - SCF (for creating biomedical resources).

The link to SCF no longer works; there is an archive of this and other work that was part of Harvard's Initiative in Innovative Computing (IIC).

So it's important to consider these initiatives at least in terms of their longevity, their scale and number of participants, as examples of funded research and as Drupal projects.

TO: EU -omics research community; if you are seeking a holistic approach for personalised medicine...

The relevance of Hodges' model as a resource in 21st century health care and research can be found within personalised medicine.

This is in addition to the need to assure holistic bandwidth within existing health care delivery, when it is person centered care that is the concern. Personalised medicine brings with it further challenges as it emphasizes the scientific, the inevitable reductionist work can potentially increase the distance between the humanistic and the mechanistic. on page 7 we read:
The figure is instructive as it shows that a holistic approach is needed to fully appreciate the challenges and opportunities presented by personalised medicine. 
'Holistic' is a much maligned word, with its fuzzy, new age connotations. Especially when the word count = '1'. Another related document also picks up the holistic call:
IMI2 will deliver tools, methods and prevention and treatment options (directly or indirectly) that will progress the vision of personalised medicine and prevention. Through providing the framework required to support collaboration between scientists, regulators, HTAs, patients and healthcare providers, IMI2 will ensure that research is translated into implementable solutions to current healthcare challenges. Solutions that are not purely focussed on the development of new medicines, but that provide a holistic personalised healthcare package as well as maintain people healthy and productive through out their lifetime. Reclassification of diseases based on their root cause and not symptoms will help addressing unmet needs even in areas where a range of options exist but patients do not respond, because their symptoms are misleading therapy choices.

On page 10:
A sustainable healthcare system is a holistic one in which the patients are responsible for their wellness and quality of life; physicians, therapists, nutritionists, community carers, and all other actors in the value chain are motivated to this goal; delivery of care takes into account patient beliefs, values and both rational and irrational behaviors; the care is affordable to both public and private payers and promotes health; sustainable businesses can thrive; and the education, prevention and management of chronic conditions are aligned to achieve this goal.
The focus of these documents is -omics and the development of new medicines. By its very nature this research, data and knowledge lies deep within many sciences: new sciences no less. This recognition of the need for holistic approaches and perspectives is still very encouraging. As the first document on personalised medicine notes, future treatments must be from "bench to bedside". We can equate this as "mechanistic to humanistic", but only as long as the patient in that bed is a person and not just viewed as a diagnosis with an associated -omic profile.

Drupalcon Portland session: Collaborative Learning Systems in Drupal & tpm

I'm still following Drupal here in NW England and around Europe since version 4.7. The next Drupalcon is in Prague next month and I'm all set. As what seems a lifelong learner of Drupal there are usually many sessions of relevance to me. There's repetition creeping in too (at Munich last year), which is a good sign for me. Perhaps it's a sign of Drupal's maturing but there are sessions of specific interest in health and education. This week the program for Drupalcon Prague starts to really emerge.

Until the 13th when the program appears, there was an educational-learning example from Portland in May 2013 embedded below, by Avram Sand and Fabian Franz. The introduction for which begins...
How can you use Drupal to add value to a Learning Management System? In this case study we will show how Trellon helped Population Services International (PSI), a leading global health organization, build a collaborative learning environment using common Drupal modules.


There are some great pointers in this session, modules to consider and insights into functionality.  Whatever TLA is employed - LMS (learning management system), VLE (virtual learning environment), TEL (technology enhanced learning) ... the lessons are transferable.

In the latest edition of the philosopher's magazine tpm 3rd Quarter there's a paper on TEL by David Mossley and Clare Saunders: A revolution in philosophy teaching? (pp. 40-45). Mossley and Saunders argue the case for tempering our expectations of TEL, framing their article around constructivism:
... TEL, presents entirely new and exciting learning environments which students can engage with in more ways, with greater self-motivation, and with more creative possibilities than ever before. If constructivist pedagogy is right, thr more "engagement" with a perfectly modelled environment of ideas in a virtual space a student can enjoy, the higher the quality of their learning experience will be; for they will be presented with just the "optimal" set of experiences needed to create their won meanings appropriately. However, once we start to analyse what the word "appropriately" might mean in this context, we suggest this starts to point us toward a different way of thinking about epistemology and pedagogy. p.42
More positively, they conclude that - technology enhanced learning:
... can and does furnish new spaces and opportunities for social learning - it opens up new avenues for us both to practise and enhance further our philosophical knowledge and capabilities and (thereby) to co-create the future of philosophy and of knowledge more broadly. p.45.
Mossley and Saunders also refer to Hegel's idea of Bildung:
describing education as the bringing into harmony of all one's thoughts, experiences, activities and practices in existing contexts; that is, education as shared approaches to ways of making meanings, of gaining competence, based on the meanings explored by others before us. p.43.
Here is another avenue to support the role of Hodges' model in education and practice.

Each issue of ERCIM NEWS is a gift and for me it does not have to be a special theme that speaks directly to the health care, informatics or education communities. I have followed this publication and posted on W2tQ about many fascinating new issues. I'm sure it is over a decade ago since I also subscribed to Technicity, a magazine from Daimler. I bring the two publications together for two reasons (not due to sponsorship - how I wish):

Firstly, this is such a difficult time socio-economically in the EU, evinced in the proportions of younger adults who are unemployed. Publications like this show the opportunities for young people who aspire, but to do so they need the support and opportunities. Safety, design, materials science, energy systems and storage, city transport infrastructure and planning, security, nanotechnology, communications, vehicular informatics ... the possibilities for youths are amazing. In turn, however we need to recognize and respond to the employment potential of those whose path is not primarily academic and research based.

Secondly, in community mental health nursing services for older adults we often come across people who are still driving when their competency is most likely compromised. They should report an admission to hospital, prescribed medication or a specific diagnosis to the DVLA. ERCIM and Technicity both presage the arrival of autonomous vehicles. At present of course an 'intelligent car' depends on the intelligence of the driver. Which is why we get so annoyed when someone drives like an *^%$£!

Driving is a great responsibility and privilege. It is central to who we are, how we define ourselves, our independence, sustaining real social networks and - in my case livelihood. I recognised decades ago there will come a time to hand over the keys. Doing so gracefully, informed by insight and self-awareness is the trick. I don't envy family doctors who have to deal with this when a patient  insists that they are safe behind the wheel. Sometimes we are ageist, hoping that a meeting with their doctor of some 20-30 years might carry some gravitas that their generation will still respond to. Add to this though that the person concerned is not always elderly. It can be very difficult for the driver (person, patient) concerned and their families too. Amid all this, as a community nurse you are trying to establish trust, empathy and rapport. Each time this issue is encountered: it is novel.

I don't want to suggest that suddenly the person behind the wheel is 'unintelligent', far from it. Evidence shows that a diagnosis of dementia does not immediately mean surrender of a driver's license. Behind dementia, however lie altered faculties that add up to more than synonyms: cognitive impairment, impaired perception, recall, and decision making, reduced concentration and attention. I thought there might be something in this issue of ERCIM related to dementia; monitoring the physiological state of the driver, reactions, attention, stress levels, eye movement. Is there a signature for health in driving behaviours?
Unlike other methods of detecting tiredness, the Mercedes-Benz system evaluates a series of indicators in order to assess the alertness of the driver and to recognise the gradual transition between being awake and being tired. AME.info
Our sense of being, of personhood is heightened by movement. When do we feel most alive? What is the most important factor in health and well-being? There are moments when we are stationary and time passes us by. There are other occasions when we move through time. We literally project our ego through time. We enjoy sharing with friends where will be in the future and when we can also suspend time. Cars and air travel extend these abilities, AND mobilities.

The danger is when we pass through not only time but distance at speed without a reliable sense of judgement, without a wholly coherent sense of self and responsibility.

In the not too distant future all this of course could lead us to a new definition of self-care at least in this context?

Dem@Care Summer School on Ambient Assisted Living

We are pleased to announce the 1st Dem@Care summer school on Ambient Assisted Living. The DemAAL summer school is primarily intended for postgraduate (PhD or MSc) students, postdocs and researchers investigating clinical and technical aspects related to Ambient Assisted Living (AAL) technologies for remote health management, ageing well and independent living. Leading researchers from academia and industry will cover theoretical and practical aspects pertinent to pervasive and ubiquitous computing technologies for Ambient Intelligence (AmI) applications, while special focus will be given on the role and opportunities of such technologies for dementia management.

Lecture material will be augmented with hands-on practical sessions. Participants will be provided with electronic versions of all programme lectures and all necessary tools and environments for the hands-on sessions. PC access with all tools pre-installed will be available on site as well. In addition, participants will have the opportunity to present their work and obtain feedback during a dedicated poster session intended to further facilitate interactions and the exchange of ideas.

Modern, industrialized, society is heavily dependent on ubiquitous, cheap energy, which we expect to be readily available, not to be polluting, and to be convenient to use.
Since the invention of the steam engine by James Watt, this paradigm has lead towards tremendous improvement of life quality in the developed world, and developing countries eagerly aspire to similar energy standards.
However, the price of this hunger for energy is increasing. Fossil fuel resources such as oil or gas are becoming harder to explore, even leading to environmental disasters as with the Gulf of Mexico oil-platform, recently. Exploration of shale gas (“fracking”) causes negative environmental impact, beyond the well-known CO2 problematics. Besides the sheer availability of fossil energy, associated CO2 emissions have caused wide-spread concerns about impacts on climate and on human health (fine particle emissions).

Thinking outside the box - meeting the needs of people living with dementia and their carers
------------------------------------------

Dementia is a major challenge facing our health and social services over the next 20 years.  This has now been recognised by the government and significant resources are being directed towards early diagnosis, new treatments and management of people with dementia.

This funding will only be effective if we can develop news ways of supporting and managing people with dementia and there carers.  This will require a multidisciplinary team approach to problem solving and service delivery.  Health and Social Care Practitioners are familiar with the concept of multidisciplinary team working but often lack insights into dynamics of group working. We need to ensure that the interventions are effective and consider outcome measures that are appropriate for a long term neurodegenerative condition that are appropriate for patients and carers.

This event aims:

- to help researchers, health and social care professionals to think differently about meeting the needs of people who are living with dementia
- to consider the range of outcome measures that might be used to assess the benefit of an intervention
- to make researchers aware of the challenges and opportunities of multidisciplinary working
- to inform new researchers of the needs of people with dementia
- to trigger ideas for new research and provide a forum where participants can develop potential proposals

Please note that this event has limited places to ensure that representatives from several disciplines have the opportunity to participate.

Keeping safe and maintaining independence: older people and sight loss

Invitation to take part in a study

I am sending some details passed onto us by one of our members, based at TRL:

We are currently undertaking a European study regarding older road users and what different countries do about supporting mobility. The aim of our project is to investigate travel patterns and road safety amongst older road users across Europe, see how they are changing and look at what work is being undertaken to support improving mobility. As part of this we are looking to undertake interviews with possible major players as to what they are doing. Would anyone be interested in taking part in a telephone interview with one of our researchers on this topic?

Recent Highlights

Falling off the Bandwagon: Sustaining digital engagement by older people - a series of consultation events have recently been undertaken exploring potential solutions to the challenges faced by older IT users. We were delighted that The Princess Royal presented a keynote address at our recent 'Showcase world class occupational therapy research to meet the needs of an ageing population' event. Associate Professor Fran Biley - Bournemouth University

It is with great sadness that this week I learned of the passing of Fran Biley. In 2003 I contacted Fran having heard he had worked up in Lancashire in the early 80s as a charge nurse on a then new elderly mentally ill unit. I could not resist the possibility that here was someone who perhaps knew of Hodges' model or had used it.

There were no major leads, but Fran always encouraged me in my preoccupation with Hodges' model. I did not have a sense that I was just being indulged, or that Fran was purely exercising good manners which I'm sure he had in abundance. Fran also provided support indirectly and so I will miss both his positive words and observations.

There may be some communications on the old PC to recover, as I've enjoyed following Fran's contributions over the years. The brilliant video on students and nursing theory which I posted here on W2tQ in 2010 and Fran's missives to various lists over the years, including:

Didn't Liam mention nursing in the original post somewhere? What about Peplau, Orlando, and Travelbee and perhaps a host of others who produced buckets full of evidence of whatever flavour you'd like to choose; and Barker as well of course, not added here as an afterthought, more as emphasis.

And as for contemporary, Orlando's book was published in ?61, and Travelbee died in 73 (I think??)....and we all (should) know Peplau's history (perhaps...)...and FNs Notes on Nursing is still on and off my book shelf like a yoyo.

Or is it that we've been influenced by psychologists, psychiatrists, sociologists and etc to such an extent that these people (and nursing per se) have clearly disappeared from the horizon, or where never there in the first place?

I will keep going Fran! Many thanks.
